Liability waiver 免责声明 All persons participating in the 2026 Fall Semester Orchestra Program shall be deemed to have waived all claims against NACYO, its board members, music teachers, staff members, assistants, and chaperons for injury, accident, illness or death occurring during or by reason of the Program. Medical consent 医疗授权 In the event of illness or injury, I consent to whatever x-ray, examination, anesthetic, medical or surgical emergency transportation, hospital care, or dental diagnosis or treatment from a licensed physician, surgeon or dentist as deemed necessary for my child's safety and welfare. I understand the resulting expenses will be my responsibility.
